As reported by the Nevada Highway Patrol, the accident happened around 6:45 p.m. on the northbound lanes of Interstate 15, just above the Interstate 11 HOV ramp close to Charleston Boulevard. Investigators found that Berry was operating a 2006 silver Honda Accord south on the northbound side of I-15, driving against the flow of traffic. This wrong-direction maneuver culminated in a catastrophic multi-vehicle crash.

The incident involved at least five automobiles in total. Along with Berry's Honda Accord, the crashed vehicles included a 2023 Tesla Model 3 sedan, a 2005 Chevrolet Tahoe, a 2008 Toyota Camry, a Ford Bronco, and a Nissan Altima. The impact from the collision caused extensive damage to numerous vehicles and necessitated a significant emergency response from police, fire services, and medical teams.

Berry was pronounced dead at the site due to the injuries he sustained during the collision. Three other people involved in the crash were taken to a hospital in Las Vegas for care. Authorities indicated that two victims were in serious condition, while another had minor injuries. There were no additional deaths reported, although the event caused considerable traffic delays on I-15 as investigators worked to clear the area and reconstruct the sequence that led to the accident.

The specific reasons that caused Berry to travel the interstate in the wrong direction are still being investigated. Officials have not confirmed if factors like impairment, confusion, or issues with the roadway were involved.
