Investigators noted that the Ford Transit was heading east on State Route 125 when the driver attempted a left turn onto Macedonia Road. At that same time, Brady was riding west on his motorcycle. The motorcycle collided with the van as it was turning, causing both vehicles to veer off the road. The collision was severe, leaving little opportunity for lifesaving measures.

Emergency services arrived quickly, but despite their attempts, Brady Mentz was declared dead at the crash site.
